Photobiomodulation has one of its strongest evidence bases in exercise recovery and performance — an area of the research that's now supported by multiple systematic reviews and meta-analyses.

Reducing Muscle Soreness

Several systematic reviews and meta-analyses report that PBM, applied before or shortly after exercise, is associated with reduced markers of muscle damage and reduced delayed-onset muscle soreness (DOMS) in the 24–72 hour window afterwards. A direct comparison with cryotherapy — a widely used recovery method — found PBM performed favourably for reducing exercise-induced muscle damage across the reviewed trials.

What this means in practice: the most consistent finding is reduced subjective soreness and selected biochemical markers of damage, rather than dramatic acceleration of tissue repair. It's a modest but clinically meaningful effect — consistent with PBM's role as a recovery adjunct, not a miracle fix.

This is directly relevant to anyone training regularly, whether in a gym, a studio, or through structured sport. If you're attending frequent or consecutive training sessions, reduced next-day soreness can support more consistent training and a more comfortable overall practice.

Supporting Exercise Performance

A well-cited systematic review and meta-analysis found pre-exercise PBM associated with increased repetitions to fatigue and delayed onset of muscular exhaustion in healthy adults. A subsequent consensus paper by leading researchers set out clinical recommendations for using PBM to enhance performance and post-exercise recovery.

The proposed mechanism — improved mitochondrial ATP availability in muscle exposed to light before exertion — is biologically plausible and consistent with PBM's broader mechanistic profile (see How Photobiomodulation Works). Effect sizes in human studies are generally modest.

For athletes and gym users working towards specific strength, endurance, or performance goals, pre-exercise PBM exposure is a non-invasive, evidence-grounded addition to a performance-oriented training routine.

DOMS is a familiar experience whenever load or movement patterns increase — common across strength training, endurance sport, and structured exercise programmes generally. The meta-analytic evidence on PBM and DOMS provides a scientifically grounded rationale for using it as a recovery adjunct after training, alongside good sleep, nutrition, and rest — not as a replacement for them.
